Re-enrollment Information
Follow a few steps to make sure your child's place is secured for the upcoming year!
Back-to-School Forms
The Registration Form (found in Skyward) must be completed prior to the first day of school for each student enrolled. Minnehaha Academy uses Skyward as its student information system. An email will be sent in January of each year with instructions for completing this form. If you are having trouble logging in, please email skyward@minnehahaacademy.net.
Athletic forms must be completed by the first day of practice (if applicable).
| Form | Purpose |

| Registration | One form per student. Provides the school with your family's current contact information, parental consent to various school policies and procedures, updated health information for your student, and student transportation plans for the school year. In the case of students with two households, this form can only be completed by the primary household due to software limitations. The other parent will have read-only access to this form. |
| MSHSL Eligibility | One form per student. Provides the MN State High School League with consent to participation requirements. This form is only required for for student participating in sports. |
| Athletics Sign-Up | One form per student. Provides the school with updated sports participation intentions for your student. This form is required only for students participating in sports. |
